• water intake
• medication and supplement notes
• pain details
• morning stiffness information
• daily factors
• personal notes
• food and nutrition records
• calories entered by the member
• caffeine intake
• movement and activity records
• rest and recovery records
• weight entries and weight trends
• appointment information
• reminders
• appointment preparation notes
• visit notes
• information included in Provider Visit Summaries

This information is entered voluntarily by the member.

5. Local Browser and Device Storage

The SRC Lupus Wellness Tracker™ is designed so that tracker entries may be stored locally in the member’s browser or device rather than being stored as part of the member’s Shopify order history.

Member-entered tracker data may therefore remain on the browser or device being used.

She-Rise Collections may not have direct access to, possession of, or the ability to recover information that exists only in a member’s local browser storage.

Tracker information may be lost or become unavailable if the member:

• clears cookies, browser history, cache, or website data
• uses private or incognito browsing
• changes browsers
• changes devices
• resets a device
• blocks browser storage
• removes locally stored website information

Members should maintain their own copies of any information they consider important.

6. Customer-Specific Tracker Access

The tracker may use information associated with the logged-in Shopify customer account, such as a customer identifier, to help separate tracker storage between customer accounts using the same browser.

Members should use the same Shopify customer account and email address associated with their membership.

This does not mean that member-entered tracker information is automatically added to the member’s Shopify customer profile or order history.

7. Membership and Appstle Information

She-Rise Collections uses Shopify and Appstle Memberships to manage membership purchases, recurring billing, membership plans, and access.

Information used for membership administration may include:

• customer name
• email address
• Shopify customer ID
• membership plan
• billing frequency
• subscription status
• renewal information
• customer tags associated with membership access
• order information

Appstle and Shopify process information according to their own privacy practices and terms.

8. Wellness Information Is Not Automatically Shared With Facebook

The SRC Lupus Wellness Tracker™ may provide a link to the SRC Lupus Community™ hosted through Facebook or another external community platform.

Tracker entries are not automatically transmitted to the Facebook group simply because a member opens or participates in the community.

If a member voluntarily posts information on Facebook or another external platform, that information becomes subject to that platform’s privacy practices and settings.

Members should avoid posting information publicly that they wish to keep private.

9. Provider Visit Summaries

A member may choose to generate or share a Provider Visit Summary using information the member entered into the tracker.

The decision to share that summary with a healthcare professional or another person belongs to the member.

Provider Visit Summaries are not automatically transmitted to healthcare providers by She-Rise Collections.

The Provider Visit Summary is not an official medical record created by She-Rise Collections.

10. Medical and Wellness Information

The SRC Lupus Wellness Tracker™ is intended for personal wellness organization and tracking.

She-Rise Collections does not use member-entered tracker data to provide:

• medical diagnoses
• medical treatment
• medication instructions
• emergency medical services
• predictions of Lupus flares
• predictions of disease activity
• professional medical assessments

Members should contact an appropriate healthcare professional for medical questions or concerns.

11. Cookies and Similar Technologies

Our website and third-party service providers may use cookies and similar technologies for purposes such as:

• maintaining website sessions
• customer account functionality
• remembering preferences
• shopping-cart functionality
